Best-fit referrals

  • The plan is understood clinically but not workable at home
  • Hospice is being discussed, delayed, or refused
  • Family conflict is blocking communication or follow-through
  • A caregiver is overloaded or nearing collapse
  • Repeated hospitalizations suggest a larger goals or support gap
  • Discharge is near and the family is not organized
  • The patient or family needs more preparation than the visit allows

Sample referral language

“This family understands the medical facts but is struggling to organize the next step, communicate together, and prepare for the transition ahead.”

What JourneyWell can take off your plate

  • Prepare families for more productive appointments
  • Reinforce, not reinterpret, information from the clinical team
  • Map caregiver capacity and practical barriers
  • Facilitate non-clinical family preparation and communication
  • Help families compare resources and prepare agency questions
  • Support follow-through between encounters

What JourneyWell does not do

  • Diagnose, prescribe, triage, or give medical advice
  • Replace hospice, palliative care, nursing, social work, or other clinicians
  • Provide psychotherapy or legal advice
  • Resolve legal disputes or determine decision-making authority
  • Offer emergency, crisis, or on-call clinical coverage
  • Promise adherence or a particular outcome

What to expect

A clear, bounded handoff.

How handoff works

With appropriate permission, the referrer shares a concise reason and preferred contact path. JourneyWell contacts the family, confirms fit and scope, and establishes communication boundaries. Updates return only with consent.

Lead time and urgency

Earlier is better, especially before discharge or a major meeting. Urgent but non-emergency situations may be accommodated based on availability. Medical urgency remains with the treating team; emergencies belong with 911 or local emergency services.

Someone by your side

Extend support beyond the clinical encounter.

Refer when a family needs time, preparation, organization, or communication support to move forward safely and thoughtfully.
