When Repeated Hospitalizations Are Telling You Something
Step back from the latest crisis to examine the pattern, goals, and supports around it.
General educational information · Updated September 2026
The real challenge
Each admission may be treated as an isolated event even when the pattern suggests changing illness, unmet support needs, or a care plan under strain.
What may be happening
- The underlying illness may be progressing or harder to manage at home.
- Medication, follow-up, equipment, or caregiver gaps may be contributing.
- The current plan may no longer match the patient’s goals.
What families can do now
- Create a timeline of admissions, triggers, and what changed afterward.
- Ask for a goals-of-care conversation rather than only another discharge plan.
- Review whether palliative care, home support, or hospice information is appropriate.
- Discuss what outcomes would make another hospitalization helpful or burdensome.
Questions to bring to clinicians
- What pattern do you see across these hospitalizations?
- Is the illness changing in a way we should plan for?
- What support could reduce avoidable crises at home?
When JourneyWell support can help
JourneyWell helps families see the whole pattern, prepare questions, and translate the next discharge into a practical plan.
Urgent and emergency note
New or severe symptoms require the treating team or emergency services; do not delay urgent care while reviewing the larger pattern.
